Health Home Care Management for Medicaid Beneficiaries

Health Home Care Management is a service model designed for Medicaid beneficiaries to ensure effective coordination and management of care. Circare Care Managers assist families with referring and linking to community resources for both immediate and long-term supports, helping to minimize emergency room visits, hospitalizations and out-of-home placements.

Eligibility Criteria

Medicaid beneficiaries between the ages of 4 and 21
AND
who have severe emotional disturbance OR HIV/AIDs OR complex trauma OR two or more chronic health conditions.
