Adult ACT RN

F/T Assertive Community Treatment (ACT) RN works with a multidisciplinary team to provide comprehensive community based mental health treatment/case management services to individuals with a severe  persistent mental illness (SPMI). This is a F/T position for 40 hours per week:  Monday to Friday 8:00 to 4:30. Telework options for some hours are available. 

Weekends and Major Holidays off. 

Salary – $35 -$36 dollars an hour

Eligible for Sign on Bonus

Principle Duties and Responsibilities: 

  • Utilizes Person-Centered Practices in treatment engagement 
  • Organizes and manages the system of medication delivery to the clients and medication administration under the direction of psychiatrist 
  • Provides medication management and planning/coordination, including injectable medications, to clients who are in the community 
  • Provides staff education to maintain familiarity with program policies and procedures regarding medication administration, management, and secure record storage 
  • Provides on call crisis intervention by covering night time hours and weekends on a rotating basis 

Job Requirements: 

  • Licensed Registered Nurse

Assertive Community Treatment (ACT) Housing Specialist

Salary Range- $47,000-$49,000 per year

The ACT Housing Specialist will work with the ACT Team, under the supervision of the ACT Team Leader, to provide clinical and care coordination in the evidence-based Assertive Community Treatment Team model. This position is an onsite/community-based work environment.  

Principle Duties and Responsibilities:

  • Charting Manager, assumes primary responsibility for clinical records, assessments and treatment planning, documentation of client progress, clinical case management.
  • Consultation with community agencies, inpatient services, and families to coordinate the treatment process.
  • Crisis management including after hour on-call duties.
  • Responsibility for investigating housing availability based on individual needs and incorporating housing services into the service plan.
  • A strong commitment to the right and ability of each person with a severe and persistent mental illness to live in normal community residences; be employed; and have access to helpful, adequate, competent, and continuous supports and services.
  • Other tasks as assigned.

Job Requirements:

  • Bachelor’s degree in human services and at least 1 year of experience in housing services with responsibilities for integrating housing services into a service plan.
  • A Master’s degree in human services with current/valid licensure in a respective field is preferred as is a strong clinical skill set;
  • a valid driver’s license is required.

Adult Health Home Care Manager

Salary- $38,000- $40,000 per year

F/T Health Home Care Manager works with and oversees the process with individuals to develop and maintain a comprehensive plan for accessing and using services and supports necessary for a person’s health.

Principle Duties and Responsibilities

  • Assures that Health Home (HH) enrollees receive needed medical, behavioral, and social services with the overarching goal of improving individual health outcomes
  • Assumes overall responsibility and accountability for coordinating all aspects of the individual’s care
  • Provides Care Management Services at various Community locations and Member’s homes
  • Leads multidisciplinary teams of medical, mental health, chemical dependency treatment providers, social workers, nurses and other care providers toward assuring enrollees receive services
  • Explores with the enrollees their immediate needs and their longer term goals and vision for their recovery
  • Assesses needs, monitors progress with enrollee/legal representative and care team, modifies or updates the care plan or goals
  • Assists in scheduling and keeping appointments, advocates and arranges for needed services and monitors delivery of services
  • Evaluates care needs at transitions, arranges safe transition plans, updates care team, updates information with providers and care plans
  • Activities including promoting self-management, facilitating family/legal representative meetings, seeking peer supports and educating enrollees of their rights
  • Identifies community networks and supports that will be utilized to address needs

Job Requirements

  • Master’s or Bachelor’s degree in healthcare or human services. Two (2) year’s experience in providing services to individuals diagnosed with a serious mental illness, a substance dependence, or chronic health conditions, OR
  • CASAC. Two (2) years of experience in providing services to individuals with a substance dependence/co-occurring disorders, OR
  • Bachelor’s degree or higher in ANY field with either: three (3) years of experience, or two (2) years of experience as a Health Home care manager serving the SMI or SED population
  • Must have a valid Driver’s license and own transportation
  • Multilingual a plus

HBCI Interventionalist

Salary- $58,000 per year

F/T Interventionist works in a home and community-based program for children with behavioral health needs. The interventionist must be a Licensed Mental Health Professional. The Interventionist will provide intensive, short-term therapeutic treatment interventions to children and youth at risk of out of home placement, psychiatric hospitalization, and children in transition from hospital or residential setting back into their home. Services are provided in environments that are most compatible with the family’s needs including home and community-based settings.

Principle Duties and Responsibilities:

  • Utilizes an array of intentional strategies to develop a positive working relationship with child/family
  • Completes initial assessment for the purpose of immediate and longer-term planning, including safety and wellness planning
  • Provides treatment based on evidence-based practice model that is utilized by the program
  • Actively coordinates services in all systems relevant to the child/family’s success
  • Fosters community connections based on person’s strengths, skills and interests
  • Makes proactive contacts with emergency and inpatient services to plan and reduce time in these settings
  • Teaches family skills through education, modeling and coaching
  • Provides in home short-term treatment to address immediate needs and support family in longer term treatment as deemed appropriate
  • Works together with family to help identify potential barriers and challenges that may arise and promotes continued success by developing a plan that will address these concerns

Job Requirements:

 

  • Licensed Mental Health Professional (LMHP) preferred. An unlicensed Master’s level applicant with at least one year experience in children’s community based mental health may be considered

HBCI Family Peer Specialist

Salary- $37,440 per year

F/T Family Peer Specialist in a home and community based program for children with behavioral health needs. The Family Peer Specialist will provide short-term support to families/caregivers of children and youth at risk of out of home placement, psychiatric hospitalization, and children in transition from hospital or residential setting back into their home.   • Facilitate meetings between families and service providers.
  • Support families to advocate on behalf of themselves to promote shared decision-making.
  • Prepare families for meetings and accompany them when needed.
  • Consult with families and providers to ensure that the family’s perspectives are included in all planning and decision making.
  • Utilize the family’s knowledge of their community in developing new and supportive relationships.
  • Provide individual parent skill development related to the behavioral and medical needs of the child.
  • Support the parent in their role as their child’s educational advocate by providing: information, modeling, coaching in how to build effective partnerships, and exploring educational options with families and school staff.

Qualifications:

  • Must be at least 18 years of age.
  • Must be a parent or caregiver of a child with a history of emotional and/or behavioral problems (parent or caregiver is defined by OMH as a parent, foster parent or other family member with direct responsibility for the care of a child with a diagnosis of emotional disturbance.) or complex medical needs. AND will have experience working with children and families in either the OMH, OPWDD, or other community system.
  • Must pass NYS Child Abuse registry background check and fingerprints.
  • Applicant must possess a credential as a family peer advocate, recognized by the NYS Office of Mental Health
  • Must have strong interpersonal skills, demonstrate effective conflict management, problem solving skills, and sound judgement.

    F/T Peer Specialist develops effective partnerships with individuals in order to identify a Path toward Mental Health and/or Substance Abuse Recovery. The process will support the development of skills in self-advocacy, choice and empowerment.

    Youth ACT Family Peer Advocate

    Be a part of a ground-breaking new program! Join Circare’s Youth Assertive Community Treatment (ACT) Team, serving families and youth in Onondaga County. This multi-disciplinary team will work with children ages 10 to 21 and their families to provide intensive intervention to support complex needs, as well as keeping children at risk of hospitalization or residential care in their homes.

    Circare is seeking a credentialed Family Peer Advocate for the Youth ACT team, who will provide formal and informal support, based on lived experience, to families who are raising a child experiencing social and emotional challenges. The position is full-time, during regular business hours, M-F. Circare is looking for a candidate interested in community-based work and who is committed to supporting children with complex mental health needs and their families.

     Salary- $35,000-$38,000 per year

    This position is eligible for a sign-on bonus.

     

    Qualifications

    • Possess a credential as a family peer advocate, recognized by the NYS Office of Mental Health
    • Lived experience raising a child with serious emotional disturbance and familiarity with associated challenges
    • Have a high school diploma, high school equivalency or a State Education Commencement Credential, at minimum
    • Knowledge of available community resources for children and families
    • Excellent communication skills
    • Experience using electronic health record programs

    Youth ACT Licensed Mental Health Professional

    Salary- $52,000 – $55,000 per year

    Circare is seeking Licensed Mental Health Professionals who will provide clinical treatment to enrolled Youth ACT children, in collaboration with medical professionals and the 8-person Youth ACT Team. This community-based program position is full-time, during regular business hours, M-F, with periodic after-hours and weekend on-call duties. Circare is looking for a candidate who is committed to supporting children with complex mental health needs and their families and is experienced and comfortable with providing services in the home and in the community.

     

    This position is eligible for a sign-on bonus.

     

    Qualifications

    • A NYS Licensed Psychologist, Licensed Clinical/Masters Social Worker, Licensed Marriage and Family Therapist, Licensed Mental Health Counselor, or Licensed Creative Arts Therapist
    • Prefer experience with multi-disciplinary in-home and community-based services
    • Strong background in recovery-based and person-centered behavioral health services
    • Excellent verbal and written communication skills
    • Proficiency in Excel and other Microsoft products

    Psychiatrist

    The Contractor will provide Psychiatric services for 14 hours per week to clients of the Circare Assertive Community Treatment Team (ACT) as part of the licensing requirement, with a total of 28 hours of prescribing time supplemented by a Nurse Practitioner who is employed or contracted by the agency. The psychiatrist could also provide supervision to an NP for minimal hours per month if Circare is granted a waiver for PNP to provide the hours in lieu of the 14 hour requirement.

    For purposes of this position the hours are negotiable at a mutually agreed upon time that meets the needs of the team.

    Salary – $190-$260 per hour

    Services (deliverables) to be provided

    The contractor will provide services in compliance with the Assertive Community Treatment Evidenced Based Model. Services are delivered predominantly (90%) in the community at person’s residences, other providers, and/or other locations in the community.

    1. Conducting psychiatric assessments including history, course of illness, and response to treatment, mental status exam, and diagnosis.
    2. Prescribing psychotropic medications and conducting regular assessments of therapeutic responses and side effects and educating clients regarding medications; assessment of co-morbid physical health conditions that may affect the client’s overall health.
    3. Providing crisis assessment and management of clients; including DCS orders
    4. Assisting the team leader in training staff in knowledge and skills basic to treatment of persons with severe and persistent mental illness.
    5. Completing documentation per ACT policy and regulatory requirements in a timely fashion that is accurate to services provided; especially when insurance will be billed.
    6. Psychiatrist – Must be currently licensed as a physician by the NYS Education Department and certified by, or be eligible to be certified by, the American Board of Psychiatry and Neurology.

     

    Minimum Qualifications

    Board certified or board eligible in psychiatry and licensed to practice medicine in the State of New York and has strong clinical skills and experience providing treatment to persons with severe and persistent mental illnesses.

     

    Preferred Qualifications:

    (Clinical supervisory experience is desirable.) Must have a clear understanding of the characteristics and problems of adults with severe mental illnesses and be knowledgeable about the ACT model. Must have a valid New York State driver’s license.
