Circare is a New York State Department of Health Designated Lead Health Home in Central New York serving 10 counties including Broome, Cayuga, Chemung, Chenango, Cortland, Madison, Onondaga, Oswego, Tioga and Tompkins.
Circare partners with Care Management Agencies throughout the 10 counties to provide Health Home Care Management to individuals in need. Circare’s dedicated network of Care Management Agencies offer highly trained and knowledgeable Care Managers to help individuals identify and reach their goals. This is done through Care Manager support in managing medical needs, making and keeping appointments, linking and referring to community supports and collaboration with the Member’s providers.
Circare’s core values include providing person-centered services, promoting choice, enhancing community inclusiveness, respecting diversity and celebrating individuality. Currently we have more than one dozen partners.
Addiction Center of Broome County
CASA Trinity
CASA-Trinity is a not-for-profit organization that helps individuals overcome challenges and lead more productive, healthy lives. We are a community based treatment center for chemically dependent men, women, adolescents and their families, providing outpatient and specialized treatment programs. Through preventative community work, CASA-Trinity confronts chemical dependency issues at all stages, by providing education, creating awareness, and investing in the future of our community.
CASA-Trinity accepts Medicaid, Private Insurance, and private client payments. We assist those who are underinsured in obtaining the necessary coverage.
Catholic Charities of Chemung County
Catholic Charities is committed to fighting the effects of poverty and its root causes through its work. Catholic Charities provides a number of needed programs and services in this community with a priority toward the poor. We work to ensure that people have food, clothing, shelter, medical services, and the ability to achieve a decent standard of living. We offer programs that specialize in residential services, community support services, and public policy advocacy and parish services. Catholic Charities serves people of all faiths and backgrounds, does not discriminate, and is inclusive of all people. Our Board of Directors is composed of volunteers from various backgrounds. Over 90% of all money donated is spent on the needs of those we serve.
Catholic Charities of Cortland County
Catholic Charities of Cortland County (CCOCC) is a private, non-profit human service agency with a history of service provision in Cortland County since 1979. Services are targeted to those affected by mental illness and substance abuse, people living in poverty, and delinquent and at-risk youth.
Our basic services include case management, residential services, and emergency assistance. The agency was incorporated in 1925 as part of the larger seven county Catholic Charities of the Diocese of Syracuse, which includes Cortland, Broome, Onondaga, Oneida, Madison, Chenango, and Oswego Counties.

Cayuga County Community Mental Health Center
Cayuga County Community Mental Health Center’s Care Management department offers services to adults who are seeking assistance with behavioral health, medical, and/or social needs such as housing. Care Managers work with individuals to identify needs and develop a comprehensive “plan of care” to meet the identified needs. The care manager and individual also develop a “care team” comprised of various medical, behavioral health, and other providers in the community who all come together to support the individual in achieving the defined goals and in obtaining optimal health.

Circare supports individuals and their families who encounter barriers to their health and wellness, both physical and behavioral, to develop the resources to live a satisfying and naturally interdependent life. Circare’s dedicated team of Care Managers, Clinicians, Vocational Specialists, Peer Mentors and Medical Professionals are trained in a multitude of evidence-based practices proven to help people attain their desired goals and to sustain recovery and build resilience. Our core value of providing person-centered services is embedded in all of our programs and promotes personal choice, respect for diversity, individuality and community inclusiveness.
Elmira Psychiatric Center
The Elmira Psychiatric Center, located in the Southern Tier and Finger Lakes Regions of Upstate New York, provides a wide array of comprehensive psychiatric services including Inpatient care and Community Based Psychiatric Services. Inpatient programs (located in Elmira) consists of a 52 bed Adult Unit and a 12 bed Child/Adolescent Unit. Community-based services comprise the largest component of the treatment spectrum and include Assertive Community Treatment, Clinic Treatment, Community Residences, School-based Day Treatment, Drop-In Centers, Family Care Residences, Intensive Case Management, Mobile Mental Health, and Prepaid Mental Health.

Family Services of Chemung provides a range of programs for children, youth, and adults in Chemung County, NY.

HCR Care Management
HCR Care Management LLC enables the company to expand existing services to support new initiatives that align with Medicaid reform and transformation in the health care delivery system. HCR Home Care has traditionally provided nursing and other therapeutic services as a licensed and certified Home Health Care Agency.

Lakeview Mental Health Services
Formerly known as Lakeview Mental Health Services, Inc., we began in 1978 with a single community residence in Naples, New York . Today we provide residential services in a six county region serving over 1800 individuals annually. Lakeview operates more than 360 beds through varying levels of care including supervised community residences for adults, treatment apartment programs, single room occupancy program and supported housing. In addition, we provide care management services in Ontario and Seneca counties.
Lakeview is funded by the New York State Office of Mental Health, the Counties of Ontario, Tompkins, Livingston, Seneca, Wayne and Yates, through client fees and private donations.

Liberty Resources
Liberty Resources Integrated Health Care provides behavioral health counseling and medication management for children, adolescents, and adults along with primary care for adults (children and adolescent services to be available in the future).
Liberty Resources Behavioral Health team is composed of licensed therapists (social work, marriage and family therapy, mental health counseling, addiction specialists); psychiatrists; psychiatric nurse practitioners; family practice nurse practitioners, and a skilled team of customer service staff. We are a training center of excellence for students at Syracuse University and Upstate Medical Center. Our multidisciplinary approach assures that treatment services are integrated and coordinated with primary care physicians. Our holistic approach to treatment incorporates wellness and the notion that physical health is often impacted by emotional health.

Monroe Plan for Medical Care is a not-for-profit health care services organization that has been meeting the needs of our members in the Greater Buffalo, Rochester, Syracuse, and Binghamton Regions for nearly 50 years. The parent company of MP CareSolutions (Our Healthcare Management Services Subsidiary) and YourCare Health Plan, Monroe Plan provides health home services and other critical health care services to individuals covered by Medicaid, Child Health Plus and the Essential Plan.

Oswego County Opportunities is a private, nonprofit human service agency serving 30,000 people each year through more than 50 programs providing:

The mission of Oswego Health is to provide accessible, quality care and improve the health of residents in our community. The Oswego Health system includes Oswego Hospital, a 164-bed facility offering acute medical, emergency, surgical, maternity and behavioral services; The Manor at Seneca Hill, providing 24-hour skilled nursing, rehabilitation services and an Adult Day Health Services Program; as well as Springside at Seneca Hill, which offers retirement living for independent seniors.

Rehabilitation Support Services, Inc. (RSS) strives to enrich and empower the lives of individuals by providing services and opportunities for meaningful emotional, social, vocational and educational growth.
RSS meets the needs of individuals with psychiatric and substance abuse disorders. A history of disability or being disadvantaged should not preclude an individual from realizing his or her full potential or ability to lead a productive life. Through housing, employment, crisis/respite services, treatment, care coordination, socialization and wellness programs, RSS rehabilitation and recovery services provide individuals with experiences that will equip them to lead productive and fulfilling lives in their communities.
Southern Tier Care Coordination
Southern Tier Care Coordination offers care management services to eligible individuals that have Medicaid. We work with a wide variety of people throughout the Southern Tier and help to improve the quality of life for those we serve. If you have certain chronic health conditions (asthma, COPD, diabetes, hepatitis, HIV, mental health concerns, active substance use, heart disease, etc.), you might benefit from these services.
TruCare Connections
The goal of the Health Home program is to make sure its members get the care and services needed. This may mean fewer trips to the emergency room or, less time spent in the hospital. It could mean getting regular care and services from doctors and providers. Or, finding a safe place to live, and a way to get to medical appointments.
Services in this program include assistance with:
- appointments with healthcare providers (such as doctors, nurses, nutritionists, counselors, mental health providers, substance abuse providers),
- medications,
- a safe place to live,
- insurance to pay for your care and services, and/or
- transportation to your appointments.
We provide Consumer Directed Personal Assistance Services (CDPAS) as a Fiscal Intermediary. Personal Care Services: Consists of nutritional and environmental support functions. Personal care functions or both such functions means some or total assistance with personal hygiene, dressing and feeding and nutritional and environmental support functions. Such services must be essential to the maintenance of the patient’s health and safety in his or her own home, ordered by the Attending Physician; based on an assessment of the patient’s needs and of the appropriateness and cost-effectiveness of services provided by a qualified person in accordance with a plan of care; and supervised by a professional nurse.
